MACVSA Application:
The MACVSA Association application is provided in HTML format below. Print the document, complete and sign it, and send it to the address shown on the application. A specific requirement to join the MACVSA Association is graduation from a Basic Examiners Course of one of the IAVSA approved voice stress instruments: The Vipre, CVSA or FVSA.. The states within the Midwest Association's region are:
Connecticut, Delaware, Indiana, Kentucky, Maine, Maryland, Massachusetts,
Michigan, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode
Island, Vermont, and West Virginia.
